We are open to serve you, answer questions or schedule an appointment during the following hours:
Monday -Thursday: 8:30 a.m. – 5:00 p.m.
Select Fridays: 8:30 a.m. – 5:00 p.m. (Hygiene only)
Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24 hours' notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.
Forms
There are two forms we request our new patients fill out. To expedite your first visit to our office and to save you time, you can fill out the necessary forms at home and bring them on your first visit.
Once the forms are completed, print, sign, and bring to your appointment. It's that easy!

This form asks about your address, employer, and other personal information.
Medical History Form
This form asks basic information about your prior medical conditions.
